Who to refer

Any patient with a chronic, progressive or terminal medical condition who:

Feels overwhelmed, confused or fearful
Is struggling to integrate his or her medical condition
Needs psychological, emotional, social and/or spiritual support
Experiences difficulties addressing practical concerns related to health issues
Faces a change in status, condition or setting
Repeatedly needs or will need in-patient hospital care
Does not comply with your treatment plan
Will enter or is in hospice care
